    Hypertension is very common. One out of every three people in the United States  have High Blood Pressure.

    For every twenty point increase in  blood pressure  the risk of death from heart disease or stroke doubles.

    Good control of blood pressure protects the kidneys and slows the progression to needing dialysis.

Treatment of Hypertension

With usual care around half of people with hypertension are not controlled to the recommended goals.

When medications are optimized, almost every patient can have their pressure controlled.

Nephrologists (Kidney doctors) have special expertise in the treatment of hypertension, because the kidneys play an important role in blood pressure control.

What are CCM Services?

Chronic Care Management (CCM) services help manage your health between office visits. The program provides a series of non-face-to-face activities and additional services especially for our CCM patients. For example:

- You will have a dedicated Care Team that is familiar with your conditions

- We actively help you manage all your medications

- We help coordinate your care with your other doctors

- We share your health information only with other authorized providers

- Our goal is to keep you out of the hospital

- Minimize the costs and inconvenience of unnecessary visits to doctors, labs, or urgent care facilities.

CHRONIC KIDNEY DISEASE

Chronic kidney disease, or CKD, is a condition in which kidney function is gradually lost. Also called chronic kidney failure, CKD impairs the ability of the kidney to filter waste products and excess fluid from the bloodstream. People with CKD often have irreversible kidney damage.

CKD often progresses slowly, and symptoms aren’t noticeable until kidney damage is more severe. As it progresses, it doesn’t just affect the kidneys. It can cause complications in multiple body systems. Serious complications include cardiovascular disease, osteoporosis, anemia, central nervous system damage, weakened immune system, and pulmonary edema.

According to the Centers for Disease Control and Prevention (CDC) Chronic Kidney Disease Initiative, an estimated 30 million adults in the United States have CKD. Most of these cases are undiagnosed.
